File of Life

WHAT IS IT??

A File of Life is a very important tool for EMTs and Paramedic's to use in taking care of patients.  It is a piece of paper which lists vital medical information about you.  If an emergency occurs and we find our patient is unable to provide critical information, this piece of paper can do the talking for you.  It lists your medical history, allergies, any medications you take and past surgeries.  By collecting this information, EMT's and Paramedic's can best treat their patient.  It also lists your hospital preference as well as emergency contacts.  We can bring this information to the hospital with us.  

HOW IT WORKS??

Place your medical information on a sheet kept in a file hung on your refrigerator.  Everything you need for a File of Life may be picked up at the Hollis Fire Station or you may contact us at 603-465-6001 and we will be happy to make an appointment for our EMS personal to bring a File of Life to you and we are also happy to assist you in filling out any and all information if needed.
